Open Records Request

​What are Public Records? 

Public records are those materials prepared, owned, maintained or retained by state and local government agencies that must be made available to the public (unless excluded by any of the exemptions in the Kentucky Open Records Act). Public agencies are not required to conduct research, compile information or answer questions as part of an open records request, but only to provide existing records that have been specifically requested.

Is there a Cost for Records? 

The following fees apply for hard copies of public records. Records will be released upon receipt of payment. There is no fee for inspecting records in person.

  • $2.00 Disk of photos
  • $0.30 Digital photo (each printed)
  • $1.00 35mm photo (each, printed)​
  • $0.60 Padded mailer
  • $0.40 CD Mailer
  • $3.00 Video Tape
  • $2.10 Signature confirmation

How do I Request a Public Record? 

To inspect or obtain copies of public records maintained by the Board of Dentistry, you must make an open records request in writing to the Official Custodian of Records. Please describe the records (be specific, i.e. name, date, location) you wish to inspect or have copied. Requests should include your address, p​hone number, printed name and signature. Mail, email, fax, or hand-deliver your request to Records Custodian, Kentucky Board of Dentistry, 312 Whittington Pkwy, Ste 101, Louisville, KY 40222.

Records may be inspected by appointment: Monday – Friday 8:00 a.m. to 4:30p.m., excluding holidays.


Please visit the Disciplinary Ac​tions page; most final orders have been uploaded to our website. These records are available to the public through the Kentucky Open Records Act​.

You may request a copy of a final order of the Kentucky Board of Dentistry using our contact us form. Please include the License Number you are requesting information about in the Comments field to avoid a delay in processing time.
