Reinstatement

To apply for a reinstatement of your Kentucky license, use the application and instructions below if your Kentucky license was active within the last five years (if it has been longer than five years, use the application by credentials or exam).

International Dentists


Requirements for Kentucky dental license applicants who are graduates of a non-CODA accredited program include two years post graduate training in general dentistry at a Commission on Dental Accreditation accredited program.

Requirements for Kentucky licensure:

Charitable Temporary Dental License

 

Pursuant to KRS 313.254, a temporary license may be granted to a dentist or dental hygienist who holds a valid license in another state to provide charitable dental care to indigent populations in Kentucky.  Requests for temporary charitable licenses must be sent to both the Kentucky Board of Dentistry and the Kentucky Department for Public Health Oral Health Program within the Cabinet for Health and Family Services.

The Kentucky Board of Dentistry must receive the request at least 30 days prior to the date of trip and must include:

Upon approval, the Board of Dentistry will provide a letter granting temporary licensure valid up to seven days.  The temporary licensee must keep the approval letter with him or her at all times while practicing in Kentucky.

Application Q & A
 

Q: How long does it take to process my application and receive my license?

A: After the Board office has received all of the information to process your application, please allow approximately two weeks to receive your license in the mail.

Q: May I call the Board office to check on the status of my application?

A: Yes.  It is the applicant's responsibility to call the Board office to check on the status of their application.

Q: Does my dental school transcript (also national and regional exam scores) have to be sent directly from the school (or testing agency) to the Board office?

A: Yes.  The Board must receive your transcript (with final degree posted on it), as well as your national and regional exam scores, directly from the school or testing agency.

 

 

Approved HIV/AIDS Curriculums
 

You do not have to take an additional HIV/AIDS course if you graduated from one of these dental schools in the past two years:
