Limited Licenses
Faculty and student limited licenses are issued in Kentucky under the supervision of the University of Kentucky and the University of Louisville. Applicants may use the standard application for dentists and indicate the type of licensure on the front page of the application.

Student limited licenses may be issued to graduates of an approved dental school and who are enrolled in a resident or post-graduate program at the University of Louisville or the University of Kentucky. With a student limited license, the student may only practice dentistry in conjunction with the school program.

Faculty limited licenses may be issued to graduates of an approved dental school who have received an appointment as a full-time faculty member at the University of Louisville or the University of Kentucky. With a faculty limited license, the faculty member may only practice dentistry in conjunction with the school program.
