Did You Know?

Each dentist's office should have the following documentation on each staff member:

Dental Assistants

Prominently Displayed

Coronal Polishing Certificate
if certified

On File at the Dental Office
    tiny note with push pin X-ray certification
if certified:

 1)  a six-hour course in dental radiography safety AND
 2)  an affidavit signed by the supervising dentist attesting to four hours of instruction in dental radiography technique (OR a four hour course in radiography technique approved by the board)

Dental Hygienists

Current Renewal Dental License
    tiny note with push pin
Anesthesia Certificate if certified

On File at the Dental Office

General Supervision if certified:

 1)  Copy of Kentucky Board of Dentistry approved GS form with all dentists' signatures for whom the hygienist works
 2)  Proof of attendance of a Medical Emergency Course specifically approved by the board for general supervision purposes
 3)  Proof of employment of experience as a hygienist of a minimum of two years with 3,000 hours
