Renewing a dental hygiene license online is fast, convenient, and secure. The online dental hygienist license renewal application will be enabled October 15, 2008 through December 31, 2008.
All renewal applications and fees are due by December 31, 2008. The renewal fee is $80 for this biennial period (2009-2010). All hygienists who renew after December 31, 2008, will be charged a reinstatement fee of $25. When you renew, the $80 payment can be made by VISA, Mastercard, American Express or ACH debit / electronic check.
If your license number is less than 1000, please add a leading zero when you enter your license number on the next screen.

Hygiene Anesthesia Kentucky Certification |

If you are certified in Kentucky to adminster block and infiltration anesthesia and nitrous oxide analgesia, you will be asked to provide an estimate of the date you last adminstered it. Please note you should be able to verify this using patient records with a reasonable degree of accuracy. However, we do not require an exact date for purposes of this renewal.

Licensees in the Military |

Military licensees who hold a dental hygiene license from Kentucky only (and no other states) and are not practicing in Kentucky, will need to answer these questions as follows:
Are you actively practicing dental hygiene? "YES"
Are you actively practicing out-of-state or out-of-country? "YES"

HIV/AIDS Course Renewal |

Kentucky licensed dental hygienists are required under 201 KAR 8:140 to take a Cabinet for Health and Family Services approved HIV/AIDS course every ten years.
For notification purposes only, we have included the year you are required to renew your HIV/AIDS course certification on the Continuing Education page of the renewal form. It is also included on the final edit page of the online renewal form. You do not need to send any C.E. documentation to Board office in order to complete the online renewal.
Please note that if you are required to re-take the HIV/AIDS course in 2009, you must complete the approved course by December 31, 2009: Dental Hygienists due in 2009
Once you have completed an approved HIV/AIDS course, please mail the Certificate of Attendance along with your Kentucky Dental Hygiene License number to:
