Dental Lab Registration
In accordance with 201 KAR 8:016, any commercial dental laboratory operating or doing business in Kentucky is required to register with the Board on an annual basis. A dental lab is considered operating or doing business within the state if its work product is authorized by a client order originating in Kentucky, even if the laboratory itself is located outside the state. Further, registered dental labs must operate under the supervision of a certified dental technician (CDT) or a Kentucky-licensed dentist.
Dental laboratories can register online through the Board's Dental Facility Portal or by submitting application documents to the Board office. Please use one of the following options to begin the application process:
Kentucky dentists may only use commercial dental laboratories that are registered with the Board and should include the registration number of the lab on the work order. Any dentist seeking to use an unregistered dental lab should request that they register with the Board before doing business.
Registration Renewal
Dental lab renewal is now open! All dental laboratory registrations issued by the Kentucky Board of Dentistry expire on July 31 of each year. Dental labs that wish to continue doing business in Kentucky must renew their registration before then. Failure to renew on time will result in an expired registration, which can only be reinstated after paying the appropriate late penalties. The renewal portal opens annually in mid-May and runs through the end of July. Online reinstatement of an expired registration will be available for a limited time after that.
To renew your dental lab registration online:
- Open the KBD Portal Page and click on Dental Facilities to log in. If you have not set up an account, click Reset Password, and a temporary one will be sent to the email address we have on file. Please contact the office if you need to update your email with us.
- Once logged in, select the Renew Registration link on the left-hand menu.
- On the Renewal screen, click the Start Renewal button to the right of your listed dental lab.
- Complete each step until you reach the Payments page.
- On the Payments page, select the method of payment and click the Process Payment button that appears.
- A confirmation will then be displayed indicating that your renewal was submitted. An email confirmation will also be sent.
Once renewal is complete, you can download a copy of your renewed license by selecting the Certificate link on the left-hand menu and clicking the Download button that appears. You can also get a copy of your receipt by selecting the Invoices & Receipts link and clicking the Review button.
