Regulation and Application
Dental Laboratories are governed by
Kentucky Regulation 201 KAR 8:016.
Effective June 18, 2014, all dental laboratories doing business in Kentucky are required to register with the Kentucky Board of Dentistry. The required registration fee is $150 for each laboratory.
Effective June 18, 2014, all Kentucky dentists shall only use the services of a commercial dental laboratory that is registered with the Kentucky Board of Dentistry. All Kentucky dentists are required to include the registration number of the dental laboratory on the dentist's work order.
If you are a Kentucky dentist and the dental lab you wish to use is not on the list above, please contact the lab and request that they register with the Board.
2019-2020 Dental Laboratory Renewal & Registration
All Kentucky Board of Dentistry approved dental laboratory certificates will expire July 31, 2019.
You must submit the Dental Laboratory Renewal Registration Form (under forms), fee and current CDT Card or Certificate before July 31, 2019. Failure to renew the registration prior to July 31, 2019 will result in removal from the Approved Laboratory List for Kentucky Dentists. Therefore, Kentucky dentists will no longer be permitted to use your lab.

Prior Lab Registration Files
The Board of Dentistry was required under the State Records Retention Law to purge and destroy all previous Lab Registration files from 2009 and earlier. Registration of laboratories was removed from the law in 2010 when the Kentucky Dental Practice Act was rewritten.
All laboratories must fill out the new registration form and you will be issued a new Registration Number.
