Dental Lab Registration
In accordance with 201 KAR 8:016, any commercial dental laboratory operating or doing business in Kentucky is required to register with the Board on an annual basis. A dental lab is considered operating or doing business within the state if its work product is authorized by a client order originating in Kentucky, even if the laboratory itself is located outside the state. Further, registered dental labs must operate under the supervision of a certified dental technician (CDT) or a Kentucky-licensed dentist.
To register a commercial dental laboratory, open the document below and follow the application instructions.
Kentucky dentists may only use commercial dental laboratories that are registered with the Board and should include the registration number of the lab on the work order. Any dentist seeking to use an unregistered dental lab should request that they register with the Board before doing business.
Registration Renewal
All dental laboratory registrations issued by the Kentucky Board of Dentistry expire on July 31 of each year. Dental labs that wish to continue doing business in Kentucky must renew their registration before then. Failure to renew on time will result in an expired registration, which can only be reinstated after paying the appropriate late penalties. The renewal portal opens annually in mid-May and runs through the end of July. Online reinstatement of an expired registration will be available for a limited time after that.
