Fees

​Fees charged by the Kentucky Board of Dentistry are set out in 201 KAR 8:520​ and are as follows:


​​​Dentists​

  • Initial general licensure: $325 (if applied for in the first year of the biennial license period) 
  • Initial general licensure: $175 (if applied for in the second year of the biennial license period) ​​
  • Renewal of general dental license: $295
  • Renewal reinstatement of an expired license from Jan. 1 - 15: $575
  • Renewal reinstatement of an expired license from Jan. 16 - 31: $835
  • Renewal reinstatement of an expired license after Feb. 1: $1,415
  • Reinstatement of properly retired general license: $325
  • Initial specialty licensure: $100 (in addition to general licensure fee)
  • Renewal of specialty license: $50 (in addition to general renewal fee)
  • Reinstatement of properly retired specialty license: $50 (in addition to general reinstatement fee) 
  • Limited ​charitable licensure: $25​
  • Initial dental anesthesia or sedation permit: $250
  • Renewal of dental anesthesia or sedation permit: $75

​​​Dental Hygienists

  • ​Initial licensure: $125 (if applied for in the first year of the biennial license period) 
  • Initial licensure: $75 (if applied for in the second year of the biennial license period) ​​
  • ​Renewal of general dental license: $110
  • Renewal reinstatement of an expired license from Jan. 1 - 15: $240
  • Renewal reinstatement of an expired license from Jan. 16 - 31: $370
  • Renewal reinstatement of an expired license after Feb. 1: $630
  • Reinstatement of properly retired license: $125​
  • ​Limited ​charitable licensure: $25​​
  • Local anesthesia privileges registration: $50
  • General supervision privileges registration: $50
  • Intravenous access line privileges registration: $50
  • Laser debridement privileges registration: $50
  • Public health registered hygienist registration: $50

​​​Sedation Facilities

  • Initial dental anesthesia or sedation facility certification: $250
  • Renewal of dental anesthesia or sedation facility certification: $75​
  • Renewal reinstatement of an expired certificate from Jan. 1 - 15: $125
  • Renewal reinstatement of an expired certificate from Jan. 16 - 31: $175
  • Renewal reinstatement of an expired certificate after Feb. 1: $225

​Dental Laboratories​ 

  • Initial dental lab registration: $150
  • Renewal of dental lab registration: $150​
  • Renewal reinstatement of an expired registration from Aug. 1 - 15: $250​
  • Renewal reinstatement of an expired registration from Aug. 16 - 31: $300​
  • Renewal reinstatement of an expired registration after Sept. 1: $350​​

​​​General Fees

  • Verification of a license: $40
  • Licensee contact lists: $100 for non-profit use; $1,000 for profit use
  • National Practitioner Data Bank query: $25
  • Returned check or rejected electronic payment: Fee amount charged by bank

​​The payment of reinstatement fees shall not be construed to exempt licensees and other entities regulated by the Board from additional penalties associated with practicing or operating without an appropriate license, permit, or registration.
